Adult Psychotherapy Forms

 Please print out, sign, and bring the following forms with you to your first appointment:  Child & Adolescent History & Intake, Informed Consent to Treat a Minor, Social Media Informed Consent. For any other form, you may use what applies to your situation. 

Custodial parents or caregivers must provide a copy of the court custody agreement for the record due upon first visit. Caregivers must print out, read, and sign the Caregiver Authorization and Affidavit and bring with to the first appointment.
